A Guide to Leg Braces and the Footwear Problems They Create

If you or someone you care for has just been fitted with a leg brace, one of the first practical surprises is usually the shoes. The brace itself gets a lot of attention at the fitting. What happens at seven in the morning when you are trying to get a shoe on over it does not.
Different braces create different footwear problems. This guide covers the main types you are likely to encounter, what each one does, and what it means for your shoes.
This is general information from a footwear perspective, not medical advice. Your orthotist or physician is the right person to talk to about which brace is appropriate and how it should fit.
Ankle-foot orthoses (AFOs)
The AFO (ankle-foot orthosis) is the most common lower-leg brace. It supports the ankle and foot, usually running from just below the knee down under the sole. Many are prescribed for drop foot — where the muscles that lift the front of the foot are weakened, often after a stroke, a nerve injury, or in degenerative conditions.
What it means for footwear. An AFO adds bulk in two directions at once. There is a footplate under your foot, which takes up depth inside the shoe, and there is an upright section behind your calf, which affects how high the shoe can come. Most people find they need a shoe that is both wider and deeper than what they wore before, and often a half or full size longer.
The other change is one people do not anticipate: an AFO holds the ankle in position. That means you lose dorsiflexion — the ability to point your toes upward — which is exactly the motion normally used to work a foot down into a shoe.
Carbon fibre and dynamic AFOs
Carbon braces do a similar job with a thinner, springier profile. They store and return energy as you walk, which many wearers prefer for how it feels over distance.
What it means for footwear. They are slimmer than a moulded plastic AFO, so shoe fit is often a little easier. But carbon is a hard, smooth surface, and the finish matters — scratching or gouging a carbon brace is not something you want to do casually given what they cost. Anything that slides against a carbon brace repeatedly is worth thinking about.
Knee-ankle-foot orthoses (KAFOs)
A KAFO (knee-ankle-foot orthosis) extends the same idea up past the knee, adding support to the thigh. These are used where more of the leg needs stabilising.
What it means for footwear. The footwear challenge at the shoe end is similar to an AFO, but the reach problem is worse. A longer, heavier brace makes bending down to work at the shoe harder, and it makes tools that require reaching past the calf less practical.
Arizona-style leather gauntlets
These are lace-up leather ankle braces that wrap the foot and ankle closely. They are often used for conditions affecting the arch and the inside of the ankle.
What it means for footwear. Leather is the specific problem here. It grips. Where a smooth plastic or carbon brace will slide against a shoe lining, leather sticks to it — which makes getting the foot into the shoe considerably harder than the brace's size alone would suggest. Wearers of these braces often struggle more than people in far bulkier plastic AFOs.
Walking boots and post-surgical braces
Controlled ankle motion boots and similar post-surgical devices are usually temporary, worn for weeks or months during recovery.
What it means for footwear. Two things. First, the boot itself is the shoe on that side, so you are dealing with a height difference against your normal shoe on the other foot — many people use an even-up shoe balancer to avoid walking unevenly. Second, when you transition out of the boot into a lighter brace, you hit all the fitting problems above at once, without the months of practice that long-term wearers have had.
The problem they all share
Whatever the brace, the same thing happens at the back of the shoe.
The heel counter — the stiffened piece at the rear of a shoe that holds its shape — is designed to be pressed against by a heel, not by a rigid brace. When a brace is forced past it, it bends. Repeat that every morning and it eventually cracks or folds flat permanently. Once a heel counter has broken down, the shoe will not hold the foot properly and it will not go back to shape.
People often assume they are buying poor-quality shoes. Usually they are not. The shoes are failing because of what is happening at the door every morning, not because of what they cost.
What actually helps
A few things make a real difference across all brace types:
- Buy for the braced side. Fit the shoe to the foot wearing the brace, then accept a slightly loose fit on the other side rather than the reverse.
- Look for removable insoles. Taking the insole out buys you the depth an AFO footplate needs.
- Choose shoes that open wide. Full lacing or long straps that open right down toward the toe make a bigger difference than any other single feature.
- Skip slip-ons. They generally do not work with a brace, whatever tool you use. You need a shoe that opens up and then fastens down.
- Use a guide, not force. The damage happens when the brace is pressed against a collapsing heel counter. Anything that holds that path open protects the shoe and makes the job easier.
